Lost Passport -wife

I called your office and below are the followings required for making the new passport; 1) Police Report / Malaysia 2) In Vietnamese - Question, can my wife write the translation into Vietnamese by hand written or please advice. 3) 4 photos 4) Photocopy of the lost passport 5) Emploter's letter and as for me, should be Marriage Certificate, correct? Please advice ASAP because we are going to get the police report on 29/9 early morning and go to the embassy with immediate effect. My wife, Tran Thi My Thanh lost the passport in Percint 2, Putrajaya on the 25/9. Thanking you in advance and your immediate response will be highly appreciated.
